Hospital to Home Care Services in North Coast NSW

Hospital to home care transition support

Coming home from the hospital should feel like a relief. But for many families, it's when the stress really begins - managing new medications, wound care, limited mobility, and the worry of what happens if something goes wrong.

The period immediately after discharge is the highest-risk time in the recovery process. Having consistent, professional support at home from day one reduces complications, speeds up recovery, and significantly lowers the risk of being readmitted to the hospital.

We provide hospital to home care across the Northern Rivers, Clarence Valley, Coffs Harbour, Port Macquarie, Mid North Coast, Tweed Heads, Coolangatta, and Kempsey regions.

If you're based across the North Coast and Northern Rivers of NSW and your loved one is being discharged from hospital - locally or from a facility further away - we can help coordinate their return home.

What We Help With

From the moment your loved one arrives home, our Support Workers can provide:

  • Safe transport home from hospital - we can collect your loved one directly from the ward or discharge lounge
  • Personal care assistance - help with showering, dressing, and hygiene while mobility and strength are limited
  • Medication management - reminders and monitoring to ensure discharge medications are taken correctly
  • Wound and dressing monitoring - observing for signs of infection or complications and alerting your medical team if needed
  • Meal preparation - nutritious meals suited to recovery and any dietary requirements
  • Light housekeeping - keeping the home safe and clean without the patient or family having to manage it
  • Transport to follow-up appointments - GP visits, specialist appointments, physio, and outpatient care
  • Overnight or 24-hour care - for those who need close monitoring in the first days home
  • Companionship and emotional support - recovery is not only physical; having someone present reduces anxiety and isolation

Don't Wait Until Discharge Day

The best time to arrange home care is before your loved one leaves the hospital - not after.

Here's what families can do while still in the hospital:

  1. Talk to the hospital's discharge planner or social worker:  Every hospital has a discharge planning team. Ask them what support will be needed at home and whether a referral for a My Aged Care assessment can be made before discharge.
  2. Request a My Aged Care assessment: If your loved one is eligible for government-funded care, requesting an assessment from within the hospital can fast-track the process. The hospital team can flag it as urgent.
  3. Contact Ideally Home Care directly: You don't need to wait for funding to be in place to contact us. We can arrange private-pay support to start immediately on discharge day, and transition to funded care once it's approved. Call us on 0477 013 186, and we'll talk through exactly what's needed.

How Is Hospital To Home Care Funded?

There are several ways to fund in-home support after a hospital stay:

Support at Home program - The Australian Government's primary aged care funding pathway. If your loved one isn't already approved, an assessment can be requested urgently from within hospital.

Transition Care Programme - government-funded short-term care of up to 12 weeks specifically designed to support recovery after a hospital stay. Ask the hospital discharge team if your loved one is eligible.

NDIS - for people under 65 with a qualifying disability, NDIS may fund transitional support.

Private Pay - if care is needed immediately and funding isn't yet in place, we offer private-pay services with no waiting period. View our pricing schedule here.
