What Is Palliative Care At Home?
Palliative care is often misunderstood as only being for the final days of life. In reality, it can begin much earlier - from the time of a serious diagnosis - and focuses on maintaining quality of life, managing symptoms, and supporting both the person and their family through every stage of a life-limiting illness.
Receiving palliative care at home means your loved one can stay in familiar surroundings, maintain their routines, and be close to the people and places that matter most.
Most people would prefer to stay at home if they can, and specialist community providers can deliver flexible palliative care programs that meet both the patient's needs and those of their family and carers.
Ideally Home Care works alongside your existing medical team - your GP, community nurses, and specialist palliative care services - to provide the day-to-day practical and emotional support that keeps your loved one comfortable at home.
We can also provide information and advice about the Commonwealth Government “End of Life Pathway” program, which can fund extensive additional help for Clients who wish to remain at home in the last 3 months of life.

Palliative Care Vs End-Of-Life Care - What's The Difference?
These terms are often used interchangeably, but they describe different phases of care:
Palliative care focuses on comfort, quality of life, and symptom management - it can begin at any point after a serious diagnosis, alongside other treatments, and may continue for months or years.
End-of-life care is a specific phase within palliative care, focused on the final weeks or days of life, with an emphasis on comfort, dignity, and being surrounded by loved ones at home.
At Ideally Home Care, we support families through both. Whether your loved one has recently received a serious diagnosis or is approaching the end of their life, we can help. Please don't wait until a crisis point to call - early conversations make everything easier.
Support for the Whole Family
Caring for someone at the end of their life is one of the most profound and demanding things a person can do. The emotional weight, the broken sleep, the constant vigilance - it takes a toll, no matter how much love is driving it.
We provide respite for family carers - whether that's a few hours during the day, overnight support, or a longer break when it's needed. Our Care Managers keep family members informed and involved, so you're never left wondering how your loved one is doing.
After a loss, our team remains available to the family. We understand that grief doesn't follow a schedule, and we're here for as long as we're needed.
