Support at Home in the Clarence Valley
From November 1st, 2025, the Australian Government's new Support at Home program replaced the old Home Care Package system.
- If you were on a Home Care Package (Level 1–4), your funding has transitioned to Support at Home.
- If you are new to government-funded care, the process begins with a free assessment through My Aged Care (https://www.myagedcare.gov.au/)
Ideally Home Care is an approved Support at Home provider.
We help Clarence Valley residents navigate the assessment process, understand their funding classification, and build a care plan that gets the most from their budget.
Services can include personal care, domestic assistance, meal preparation, transport, nursing visits, and allied health support. Learn more about our pricing schedule here.
Already have a Home Care Package or Support at Home budget? You can switch to Ideally Home Care at any time. There is no lock-in with your previous provider.

Frequently Asked Questions
Do you provide home care in Yamba and Iluka, or just Grafton?
Yes, we support clients right across the Clarence Valley, from Grafton and South Grafton through to the coastal communities of Yamba and Iluka, as well as Maclean, Lawrence, Ulmarra and surrounding areas.
What's the difference between a nursing home and home care in the Clarence Valley?
A nursing home (residential aged care) means moving into a facility. Home care means a carer comes to you, so you or your loved one can stay in your own home in Grafton, Maclean, Yamba or wherever you live in the Valley. If you're weighing up the two, we're happy to talk through what's realistic for your situation.
What happens to my care visits if the Clarence River floods?
We plan around flood risk as a matter of course. Where roads or causeways are likely to close, we build contingency into the schedule in advance so care isn't disrupted without warning.
Can I switch to Ideally from another Clarence Valley provider?
Yes. If you're already receiving a Home Care Package or NDIS support through another provider and want to change, we can guide you through the transfer step by step.
Do you support rural properties outside Grafton and Maclean, like Copmanhurst or Nymboida?
Yes, our local carers cover the wider Clarence Valley, including smaller rural communities. Get in touch and we'll confirm coverage for your specific address.
